Step 1: Fill out an inquiry questionnaire

Begin with this simple questionnaire. This will help us match you with the best fit therapist, address logistics, and give you an opportunity to ask questions. As soon we receive it, we will be in touch with answers to your questions and next steps!

Step 2: Complete your intake forms

Once you have received feedback on your inquiry questionnaire, you can complete the intake forms. These include your basic information, HIPAA rights and protections, consent to therapy, consent to Telehealth (if applicable), and your payment authorization.

Step 3: Schedule an appointment

Once your forms are complete, you will be connecting directly with your therapist to schedule an appointment. Your therapist will be reaching out to you, but here is their contact information as well:
